A
- collect GBS
- treat either with gbs specific abx or latency
- can dc gbs specific after 48 hrs if no labor

- if no labor; no abx
- if labor or rom; give abx that cover both surgicla ppx and gbs ppx

regimens for gbs ppx (ideal)
A
penicillin g 5 million units then 2.5 million units q4 hrs until delivery
OR
ampicillin 2 gm bolus then 1 g q 4hr IV

regimens for gbs ppx (low risk pcn allergy)
A
ancef 2 g then 1 g q8hr until delivery

high risk pcn allergy gbs ppx - definition and options
A
- anaphylaxis, resp distress, angioedema, or urticaria
- clinda 900 mg q8hr until delivery (if susceptible to both clinda and erythromycin)
OR - vancomycin 20 mg/kg IV q8hr (max 2 g dose)
