HBsAg positive
Anti-HBc positive
IgM anti-HBc negative
Anti-HBs negative
chronically infected
HBsAg negative
Anti-HBc positive
Anti-HBs negative
- serologic window (recovering from acute infection)
- distantly immune, test not sensitive enough to detect very low level of Anti-HBs in serum
- false positive anti-HBc
- undetectable HBsAg and person is actually chronically infected
where is HBcAg found? where is HBsAg found?
- hepatocytes
- serum
what does HBeAg represent?
active replication/high infectivity
- associated with cirrhosis
- associated with liver cancer
vertical transmission rate (if no ppx)
- if HBsAg pos: 20%
- if HBsAg + HBeAg: 90%
- 1st trimester acute infection: 10%
- 3rd trimester acute infection: 90%
modes of transmission?
- during delivery (85-95%)
- transplacental
- close maternal contact after delivery
neonatal tx?
- Hep B vaccine
- HBIG
- Both given within 12 hrs of delivery, at different IM sites
what is the first antibody to show with hep b infection
- Anti-HBc IgM
what is then serologic window?
when HBsAg is cleared, but HBsAb not yet detectable
what is treatment for Hep A
- vaccineadn HbIg within 2 weeks; okay in preg
