What is enduring power of attorny
EPA is a legal document
1. Personal care and welfare - This EPA comes into effect only if the donor becomes mentally incapable (e.g., due to illness or injury) and can no longer make those decisions themselves.
2. Property -This EPA can be set up to start immediately or in the future when the donor becomes mentally incapable. The donor can also specify whether the attorney can act before the donor becomes incapacitated or only when incapacity occurs
What is an advance directive?
Legal document that allows a person to state their healthcare preferences in advance, in case they become unable to communicate their wishes due to illness or incapacity

What is End of Life care (EOL)?
Refers to the last days of life – when the person
is ‘actively dying’

ACP is a process of shared planning with an individual,
whanau & health care professionals.
ACP document gives people the opportunity to develop & express their preferences for future care
