Define multi-morbidity
Multimorbidity refers to the coexistence of two or more chronic health conditions or diseases in an individual, particularly in the context of long-term illnesses or conditions that persist over time. These conditions can range from physical health issues (like diabetes or hypertension) to mental health conditions (like depression or anxiety), and they often interact in complex ways, affecting a person’s overall health and quality of life.
What is involved in management of complex health states?
Multimorbidity complicates the management of individual diseases, as treatments for one condition may interfere with or exacerbate another, and the individual may face increased challenges in managing their overall health.
Complex treatment regimens: Managing multiple medications and therapies.
Increased risk of complications: Conditions can interact and worsen each other.
Patient-centered care: Requires a holistic approach that addresses the patient’s full range of health needs, rather than focusing on one disease at a time.
