- Most common form
- Results from one of several inboorn errors of purine metabolism
- Production of uric acid > excretion of uric acid
- May be inherited
- Peak onset is from 40-50

- Hyperuricemia from another disease or problem
- Does not discriminate on the basis of age

- only obvious from a serum uric acid level

A
Occurs after repeated attacks
- deposits of urate crystals form under the skin and in major organs
- renal- philic
- kidney stones

A
- History
- Joint inflammation
- Severe pain
- Typically affects smaller joints
- Renal stones
- renal insuffiencey
- Tophi (outer ear, arm, fingers )

Interventions for gout
A
- Pharmacology: colchicine and allopurinol
- take after meals
- Encourage fluids
- Low purine diet (No organ meat, shellfish , oily fish with bones)
- avoid aspirin
- avoid diuretics
- avoid stress
- stress managemnt
- alkaline ash foods
- Citrus foods
- Milk/dairy
