Aetiology?
50% have PMR
Typical presentation
50y/o woman presents with new onset temporal headache and tenderness. She also complains of fleeting blindness “like a curtain being drawn”.
What is GCA?
Granulomatous inflammation of medium sized vessels
What is fleeting blindness in GCA?
Amaurosis fugax
Branches of which artery are affected?
ECA
What is a red flag symptom and what does it mean?
Blindness
CN2 ischaemia
What is the diagnostic criteria?
Requires 3/5
- > 50
- New onset temporal headache
- Temporal tenderness
- ESR >50
- Abnormal temporal artery biopsy
Tests?
ESR
Temporal artery biopsy
US Temporal artery
What is seen on temporal artery ultrasound?
Halo sign of granulomas
Treatment?
Prednisolone
+
Aspirin
What can be used if PMR +?
Methotrexate
What follow up stuff must be done?
AAA screening = abdo USS
Yearly CxR for aortic dilatation
What differential must be considered if <55?
Takayasu’s arteritis
