GI Pharmacology Flashcards
What is the MOA of an antacid?
Neutralize HCL secreted by parietal cells
What are the key pharmacokinetic properties of Baking soda (Sodium Bicarbonate)
Water Soluble, Rapidly reacts, leaves stomach quickly
Adverse effects of Baking soda?
1 .High sodium content is bad for hypertension.
2. Metabolic alkalosis and fluid retention
Pharmakinetic properties of Aluminum hydroxide
Low acid neutralizing capacity
Adverse effects of aluminum hydroxide
- Constipation
2. Can build toxicity in CKD patients.
Clinical use of aluminum hydroxide
Used in renal failure to bind phosphates in small bowel
Name an H2 receptor antagonist
Ranitidine
Pharmakinetic properties of Ranitidine
- Inhibits gastric acid secretion stimulated from histamine, food and CN X.
- Does NOT lower LES tone or gastric motility
Adverse effects of Ranitidine
- Cimetidine blocks androgen receptors at high doses.
- Delirium in elderly
- Tolerance over time
Drug-drug interactions of Ranitidine
- Cimetidine inhibits CYP
2. Affects warfarin
Clinical uses of Ranitidine
Ulcer hypersecretory states, Zollinger-Ellison, Reflux esophagitis
Name some Proton pump inhibitors
Omeprazole, Lansoprazole, esomeprazole
MOA of proton pump inhibitors
Irreversible noncompetitive inhibition of H/K ATPase in gastric parietal cell.
Drug-Drug interactions of Omeprazole
Inhibits CYP 2C19 thus, phenytoin, diazepam, clopidogrel
Clinical uses of omeprazole
Ulcers, GERD, Zollinger-Ellison
