Where do the promotility drugs have their effect?
Fundus and antrum of stomach
What are two promotility agents that are dopamine antagonists? What is another promotility agent?
Metochlopramide and Cisapride. Domperidone.
What does metochlopramide do?
Increases motor tone in lower esophageal sphincter (prevents reflux) and stomach (improves gastric emptying). Also an anti-emetic.
Used for GERD, anti-emetic and gastroparesis
What is a side effect of metochlopramide?
Tremors
What do the anti-inflammatories end in? What are they used for?
“-salazine” or “-salamine”
Acute IBD (Ulcerative Colitis)
How is sulfasazaline broken down?
Sulfapyridine is cleaved from it to release 5-ASA
How is olsalazine broken down?
Broken down by bacteria in bowel into 2 molecules of 5-ASA
Which anti-inflammatory is used for the terminal ileum and colon? How is it broken down?
Mesalamine. Delayed release of 5-ASA.
What is prednisone used for?
IBD (acute and chronic)
What three immunosuppressive drugs are used for chronic IBD?
Azathioprine, Cyclosporine, Infliximab
What is the mechanism of azathioprine?
Purine antimetabolite; interferes with DNA synthesis
What is the mechanism of Cyclosporin?
Suppresses T-helper and T-suppressor lymphocytes
What is the mechanism of Infliximab?
Binds and neutralizes TNF-alpha
