What is Gastro-Oesophageal Reflux?
Contents from the stomach reflux through the LOS into the Oesophagus, Throat and Mouth.
Aetiology of Gastro-Oesophageal Reflux in Babies.
Immaturity of LOS allows reflux - normal, provided there is normal growth and no other concerning features.
Prognosis of ‘Healthy’ Gastro-Oesophageal Reflux.
90% of infants stop having reflux by 1 year.
Give 8 causes of vomiting in children.
- Overfeeding.
- Gastro-Oesophageal Reflux.
- Pyloric Stenosis.
- Gastritis/Gastroenteritis.
- Appendicits.
- Infections e.g. UTI, Tonsillitis, Meningitis.
- Intestinal Obstruction.
- Bulimia.
Epidemiology of Gastro-Oesophageal Reflux (3).
- Commonest cause of vomiting in infancy.
- 40% of infants regurgitate feeds to a certain extent.
- Develops typically before 8 weeks.
Risk Factors of Gastro-Oesophageal Reflux (2).
- Pre-term Delivery.
2. Neurological Disorders.
Red Flags of Dehydration (4).
- Drowsy.
- Floppy.
- Dry Mucous Membranes.
- Sunken Eyes.
What is Posseting?
Vomiting simple small volumes of milky feeds (extremely common after feeds). Larger volume regurgitation may represent GORD.
What is Sandifer’s Syndrome?
A rare condition that causes brief episodes of abnormal movements associated with GOR in infants - neurologically normal but acute dystonic reactions e.g. torticollis.
Prognosis of Sandifer’s Syndrome.
Tends to resolve as the reflux is treated or improved.
Red Flags of Problematic Reflux (6).
- Chronic Cough.
- Hoarse Cry.
- Distress, Crying, Unsettled After Feed.
- Reluctance to Feed.
- Pneumonia.
- Poor Weight Gain.
Management of Gastro-Oesophageal Reflux in Kids (5).
- Explanation, Reassurance and Practical Advice.
- Gaviscon mixed with feeds.
- Thickened Milk/Formula.
- Ranitidine.
- Omeprazole (if Ranitidine is inadequate).
Give 4 practical advice tips to parents.
- Small frequent meals.
- Encourage burping regularly to help milk settle.
- No overfeeding.
- Keep baby upright after feeding (30 Degrees Head Up).
Why should infants sleep on their backs?
Reduce risk of cot death.
Give 4 examples of thickened milk or formula alternatives.
Specific anti-reflux formulas e.g. :
- Rice-Starch.
- Corn-Starch.
- Locust Bean Gum.
- Carob Bean Gum.
