Risk factors of GDM
1) mom over 25
2) previous baby born over 9 lbs
3) 1st degree relative with diabetes
4) glucocorticoid use during pregnancy
5) PCOS
6) hypertension
7) previous perinatal malformation
8) glycosuria
9) maternal obesity
10) metabolic syndrome
ethnic groups with high prevalence of T2DM
Hispanics African americans Native americans South or East Asians Pacific Islanders
impaired fasting glucose
100-125, over 125 then diabetes
impaired glucose tolerance
140-199, after glucose challenge
GDM screen, glucose challenge
give 50 gm oral glucose then measure after 1 hour
- if less than 130 then normal
- if over 130 then give 100 gram of oral glucose in a fasting state
- measure glucose during fasting, 1 hr, 2 hr, 3 hr
GDM screen glucose challenge positive result
if any 2 are over limit then positive result -fasting over 95 1 hr over 180 2 hr over 155 3 hr over 140
maternal complications of GDM
stillbirth
preeclampsia
hydramnios
babies that are large for gestationl age
increased long term risk for mother with GDM
type 1 DM
type 2 DM
CV disease
fetal complications
shoulder dystocia
brachial plexus injury
birth trauma
increased long term risk of obesity and metabolic syndrome
