Herpes Simplex
HSV-1 = cold sores + genital HSV-2 = genital

What is the pathophysiology of genital herpes?
A
HSV enters the body through small cracks in skin/mucosa
Travels to nearest ganglion
- trigeminal = cold sore
- sacral = genital
Reactivation causes symptom flair

What are features of an active genital herpes infection?
A
Ulcers/blisters
Neuropathic pain
Flu-like symptoms
Itchy genitals

How is suspected genital herpes investigated?
A
History - ask about partner symptoms including cold sores
Viral PCR swab from open sore

How is genital herpes managed?
A
Primary
- PO aciclovir
Reactivation = symptomatic relief

How is genital herpes in pregnancy managed?
A
Before 28 weeks = PO aciclovir
If active = recommended caesarean section
