General Anaesthesia Flashcards
What are risk factors for delayed return of conciousness
What comorbidities predispose to increased risk of delayed return of conciousness
RESP
(Reduced Central Drive) OHS, COPD, intracrainal pathology, OSA
Neuromuscular Disorder
Myasthenia gravis, Motor neuron disease, Guillain Barre, Muscular dystrophy
Hepatic - acute/chronic failure
Renal - acute /chronic failure
Endocrine - hypothyroidism
Describe an approach to the patient with delayed return of consciousness
What are some of the medical, pharmalogical and surgical causes of delayed return of conciousness.
Surgical Causes
Steep Trendelenburg ( eg robot prostate leading to cerebral oedema)
Hypotonic irrigation fluids able to enter circulation (TURP) leading to electrolyte disturbance and cerebral oedema - also direct central inhibitory action of glycine
Cardiac Surgery with bypass - complications include hypothermia, haemmorhagic stroke from anticoagulation, cerebral ischaemia from microemboli,
