A

most gastrinomas are sporadic, but 25% are associated with MEN I

Gastrinoma pathogenesis?

A
  1. May arise in the pancreas or duodenum or in tissues surrounding pancreas; often
    malignant and metastasize to the liver but, histologically: rarely show anaplasia
  2. secrete gastrin causing acid hypersecretion, leading to peptic
    ulcers and pancreatic enzyme inactivation

Gastrinoma clinical signs?

A

Peptic ulcer disease (usually in duodenum) often complicated by ulcer perforation;

diarrhea,

steatorrhea,

weight loss (caused by pancreatic enzyme inactivation)

A

Surgical removal of gastrinoma; control of gastric acid secretion with proton pump inhibitors

What is Insulinoma?

A

tumor of the islet cells of the pancreas that is characterized by increased secretion
of insulin and increased C-peptide (molecule that is cleaved from proinsulin during insulin synthesis)
levels

Insulinoma associated with?

A

Whipple triad

  1. episodic hyperinsulinemia and hypoglycemia,
  2. hypoglycemic CNS dysfunction,
  3. reversal of CNS dysfunction upon resolution of hypoglycemia
