Function and Exercise Flashcards
Lancet Commission 2020 - Physical activity and exercise effect on normal adults, MCI
- SR - cognitively normal adults, global cognitive improvement for mod-vigorous resistance or aerobic exercise, no effect from yoga
- MA - MCI global cognition in intervention group with aerobic exercise improved more
Evidence regarding group and home based exercise programs
Community dwellers
Falls reduced in those who undergo program
Usually in multiple exercise categories (balance and function plus resistance) and Tai Chi alone
12 benefits of physical activity aside from those related to DM, HTN, obesity, TIA
- Increase bone density
- Decreased risk of hip fracture
- Decreased risk of falls/improved balance
- Decreased risk of frailty
- Increase muscle mass (tx/px sarcopenia)
- Improved appetite
- Decreased rates of depression
- Improved sleep
- Preservation of cognitive function
- Decreased CV mortality
- Decreased risk of cancer
- Decreased all cause mortality
6 components of an exercise program for falls prevention
- Balance challenging and functional exercises
- Sufficient intensity and duration, delivered in safe way that considers functional abilities
- Sessions 3x/wk or more
- Program should be individualized (trained professionals)
- Progress in intensity for at least 12 weeks
- Include Tai Chi +/- additional progressive resistance strength training
Canadian physical activity guidelines for >65 yo
- Mod-vigorous aerobic activities, min 150 mins/wk
- Muscle strength 2x/wk
- Activities that challenge balance
- Several hrs of light physical activity, including standing
What are the components of an exercise prescription?
- Frequency - work up to daily
- Duration - 150 mins /wk
- Intensity - mod to vigorous
- Type - aerobic
What are two physiologic causes for change in balance?
- Decreased joint proprioception
- Decreased vestibular hair cells = decreased proprioception
Four tools to assess balance
- Berg balance scale
- SPPB
- Tinetti - POMA
- TUG
- Functional reach test
8 aspects of home exercise program that increase adherence in older patients
- Educate patients pain vs. harm
- Reinforce messages to reduce fear/anxiety about pain
- Emphasize that exercise will lead to less pain, anxiety, depression
- Countering maladaptive coping strategies
- Motivational techniques to set personal goals
- Manage expectations
- Minimize pain
- Graduate program
- Provide clear instructions, printed and verbal
- Encourage to keep logs, diaries etc.
- Review at every treatment
- Tailor to the patient
- Telerehab/apps
- Pictures and videos
- Include social network
How to help discharge planning in patient post stroke, functional dependence, cognitive impairment
- Family meeting with allied health, use of interpreter
- Home OT safety assessment for modifications
- OT assessment for functional abilities/tools
- Indigenous health navigator
- Determine capacity/POA
- Close follow up on discharge/?home visit
5 components of the Functional Independence Measure (FIM)
- Self care - eating, grooming, bathing, dressing (upper and lower), toileting
- Sphincter control - bladder and bowel
- Transfers - bed, chair, wheelchair, toilet, tub, shower
- Locomotion - walk, wheelchair, stairs
- Communication - comprehension and expression
- Social cognition - interaction, problem solving, memory
