Listeriosis in pregnancy
granulomatosis infantiseptica. Infants with this disorder have disseminated abscesses and/or granulomas in multiple internal organs
Most neonates with granulomatosis infantiseptica are stillborn or die soon after birth
Neonates probably acquire infection during or after birth. Infection in the first week of life is usually manifested by sepsis, while disease manifestations after the first week are more variable and often include meningitis.
ampicillin (2 g IV every four hours)
7-day course of antibiotic therapy with oral amoxicillin (500 mg 3 times daily or 875 mg 2 times daily)
CMV vertical transmission
per trimester
Increased risk for vertical infections from 35-65% with primary
later trimesters- less symptomatic
Congential syphilis
hepatomeagly placentomegly anemia polyhydramnios hydrops -1/3 of affected fetuses are stillborn - most likely if delivery occurs within 30 days of delivery
- invasive testing not recommended
toxo transmission rates per trimester
similar to CMV increases with gestation
- u/s also similar to CMV
palo alto labs for serology!!!
congenital toxo
40% severely affected
- CP microcephaly blindness hydrocephalus epilepsy death
treatment for CAP
amoxicillin or amoxicillin/clavulanate plus azithromycin
