What sort of regiment should we use on women who are hypothalamic anovulatory function?
FSH and LH
What is Monotropins?
aka Pergonal
- with hCG for ovulation in anovulatory women for production of multiple follicles
What is urofolitropin? Used for?
aka Metrodin
- used with hCG to induced ovulation of pts with polycystic ovarian disease
usually post clomiphene citrate failed therapy
What is follitropin a and follitropin b?
alpha and beta FSH respectively
- induce follicle development and ovulation when coupled with hCG
What are the common AE’s of the tropins?
Multiple fetuses
Ovarian hyperstimulation syndrome
What is clomiphene citrate and its MOA?
Estrogen antagonist in the hypothalamus
- non- selective estrogen receptor modulator (SERM)
- thus we are blocking the negative feedback of estrogen on the Ant pit and hypothalamus
- this stimulates release of GnRH
What is Clomiphene citrate used for?
Fertility drug
- 80% of anovulatory pts will ovulate
What are the AE’s of climiphene citrate?
Multiple fetuses
- hot flashes
- ovarian hyperstimulation syndrome
What is the purpose of recombinant hCG and what is it called?
Choriogonodotropin alpha
- used to mimic the LH surge and induce ovulation
What is the MOA of choriogonodotrpin alpha?
Binds LH/CGR to induced ovulation
-stimulates the estrogen primed follicle to induced meiotic resumption of oocytes
What are ganirelix and cetrorelix?
Competitive antagonists of GnRH
- prevent endogenous LH surge
What can Ganirelex and Cetrorelix be used for?
Uterine fibroids
- fertility when used with exogenous gonadoptropins
