When would a driver contact the PD help desk?
-Assistance with fault finding
-PAE from a signal reverting to stop
-Train fault that impacts running of train
-driver error that affects train running
What part of the TMM are provided by Train Control?
Fault category

What are the categories of faults? How long can a train stay in service with each type of fault?
Maintenance fault - 90 days
Serious fault - end of day
Critical fault - out of service ASAP
When is the fault warning notice used?
When a fault occurs that impacts a safety critical component, such as:
- bogie isolated
- air bag deflated
- lateral sway fault
- red EC/yellow TECHAV isolated in end motor
- doors isolated
How are FWN’s placed in the defective train?
Yellow copies are placed in both end motors
Copies are moved if the train is divided
Fault warning stickers are used if speed restriction is applied
What are the main indications that a whistle is defective/faulty? What is a general fix for each issue?
Sounding continuously
- rapid lever operation; isolate whistle
Not sounding
- rapid lever operation; inspect whistle cocks/circuit breakers
Tone too low
- submit TMM, can still run as normal
How is a train operated when a whistle fails in service?
- departing from platforms; driver must check along the train after closing doors before departing, reopen doors for people approaching doors
- travel at reduced speed; must not exceed 15km/h in yards, stations or near line side workers
- approaching level crossing/workers; headlight high/low or hazards
What are the procedures for operating with faulty headlights?
One faulty headlight:
- no restrictions, submit TMM
Both faulty, daylight hours:
- no restrictions, but max 50km/h in MURL and tunnels
Both faulty, darkness (critical fault):
- max 50km/h between stations/crossing loops
- max 15km/h over crossings, through yards/stations
- frequent whistle usage
What is the procedure for a locked axle/wheel?
- Max of 5km/h with skidding wheel
- Train cannot be moved if lead wheel/axle is jammed (return to rear/rescue from lead)
What is the procedure for isolated bogies?
> 80% bogies:
- cut speed by 10km/h, minimum reduction of 60km/h
- can remain in service
50%-80% bogies:
- extreme caution
- detrain passengers, can be driven to siding
<50% bogies:
- train disabled
