What is the patient first review?
Made in 2009 Saskatchewan and called for changes to how patients experience the health system. Recommendation to change from system centred approach to patient/family centered care.
System centred approach 1990s?
Patient relied entirely on staff, practitioner takes charge/decides for patient, blood relatives only family, family not included in care, beneficence is practiced
Patients and family centred care 2005?
Impact of disease on patients daily life is important, involvement of caregivers, patients expertise considered, patient is member of team/can make decisions, and sociocultural and psychosocial background is considered.
People centered care 2015?
Foster respect/compassionate/competent/culturally appropriate care that’s responsive to needs and beliefs of family/patient, patient defines what family means to them, work as a team and everyone has voice in decisions
What is patient and family centred care?
grounded in mutually beneficial partnerships among heath care providers/patient/family, work with the patient rather than doing it to/for them. Provides a framework to improve care/safety/efficiency.
4 pillars of patient/family centred care?
Respect/dignity, info sharing, participation, collaboration
What’s respect and dignity?
Staff/doctors listen and honour pt/fam perspectives/choices and incorporate those into delivery of care.
What is info sharing?
Timely, complete, unbiased, evidence based info is provided to patients/families in ways they understand so they can participate.
What is participation?
Patient/families are supported/encouraged to participate in their care/decision making at the level they choose.
What’s collaboration?
Pt/fam work with care providers in professional education, delivery of services, healthcare facility design, policy and program devleopment.
What is accreditation?
Ongoing process of assessing health organizations against standards of excellence to identify what’s being done well/what needs improving.
What is people centred care?
Approach that adopts pt/caregivers/fam/community perspectives as participants in health systems that are organized around needs of people. It encompasses clinical encounters/includes attention to health of people in communities.
