Extra Rem Pros Revision Flashcards
If a patient has stopped wearing their dentures as it causes them pain or ulcers what should you do?
Wear dentures on the day of their appointment to remind them where the sore areas are
What can go wrong during denture flasking? (7)
- Contraction porosity (not enough dough in flask)
- Gaseous porosity (flask heated up too quickly)
- Granular porosity (insufficient monomer)
- Uncured chemicals (uncured monomer = irritant/toxic)
- Structure may be weaker if not cured properly, or there are cracks or porosity
- Appearance may change
- Acrylic can distort and denture may not fit well (even after adjusting)
What are instructions you should give to patients about wearing their dentures?
- Wear during the day (not at night) to avoid denture stomatitis
- Don’t wear during the day if painful/cause ulcers - Just wear on the day of review appointment to avoid unnecessary pain and remind patient of sore area(s)
.
At what stage of denture construction do you take an interocclusal record?
At jaw registartion for master cast stage
(Decide either RCP or ICP)
Talk through how can we decide where to position upper anterior teeth in an edentulous patient?
- Match denture teeth midline to midline of face (nose and between the eyes when smiling)
- Line teeth near incisal edges near (or 1-2 mm below) lower border of upper lip at rest
- Ensure a favourable Lip support (roughly a 90 degree angle between lower border of nose and upper lip profile looking from the side)
- Match long axes of upper denture teeth with patient’s sagittal plane/midline
- Look at denture gingival margins and lower border of upper lip when smiling (to see how much pink wax/acrylic is showing)
- Compare denture tooth position against natural teeth if present
What are instructions you should give to patients about cleaning their dentures?
- Scrub all surfaces gently (so does not scratch) using soft denture brush and warm soapy water (no harmful chemicals)
- Clean over a sink/bowl of water to avoid denture fracture if dropped on hard surface
- Clean twice a day and when leaving out at night
What are some implications for a patient having dentures with not enough freeway space? (4)
- Soreness under dentures
- Aching jaw muscles due to excessive OVD
- Patient not wearing denture as it is uncomfortanle
- Poor appearance (incompetent lips, excessive teeth display)
.
Why does a tooth prep need to be done prior to placing a rest seat?
- To allow for space between upper and lower teeth for adequate thickness of the rest seat
- Provide more suitably inclined bearing surfaces compared to not prepared teeth
What is an ideal amount of freeway space for an edentulous patient?
Ideally 2-4mm
Why make a patient smile when assessing tooth position of denture teeth?
Look at gingival margins and border of upper lip to see how much pink wax/acrylic is showing
How do you calculate freeway space?
What tool can be used to help measure this?
FWS = RVD - OVD
Using Willis Gauge
What are some implications for a patient having dentures with too much freeway space? (6)
- Deep creases at corner of mouth (leading to angular cheilitis/ soreness/ inflammation)
- Denture stomatitis associated with angular cheilitis
- Poor appearance (poor lip support)
- Aching jaw by overclosing due to reduced OVD
- Worn denture teeth
- Difficulty chewing
At what stage of denture construction do you add wax rims?
Usually fit framework first and then add wax rims to do jaw registration rather than make separate rims
