exocrine pancreas Flashcards
what are the two exocrine cells of the pancreas?
acinar cells - secrete enzymes and electrolytes
ductal cells - modification of fluid , add water add bicarb
what is the difference in the pancreatic fluid in relation to the flow ?
high flow - low chloride and high bicarb
low flow - high chloride and low bicarb
sodium concentration doesn’t change ALWAYS ISOTONIC - equal to plasma
what is the main stimulus of bicarb secretion from the pancreas ?
secretin
what is the major hormonal stimulus for release of pancreatic enzymes ?
CCK
as well as Ach from the vagus nerve
vagovagal reflex
what form of carbohydrates are absorbed by the intestine ?
only monosaccharides
what is the rate limiting step for carbohydrate digestion ?
intestinal brush border
what enzymes are elevated in acute pancreatitis ?
amylase
lipase - more specific
what are the enzymes secreted by the pancreas used to digest lipids ?
lipase
colipase - helps lipase
phospholipase A2
what is phospholipase activated by ?
trypsin
what form are protein digestion enzymes released as ?
released in an inactive form called zymogens
what are pancreatic proteases activated by ?
activated by brush border enzyme enterokinase
what is the pathophysiology of acute pancreatitis ?
large amounts of trypsin activated enzymes that cant leave - causes auto digestion
what conditions are associated with pancreatic enzyme replacement ?
cystic fibrosis
chronic pancreatitis
post pancreactectomy
