What able to do at each stage of repair, contraindications
Bleeding phase of repair, what exercise can be done and what does this stage entail?
No exercise. Allow bleeding to continue, control the swelling. Dont want excessive bleeding as excessive scar production (fibrin), RICE.
Entails- first few hrs after injury, 5 cardinal signs of inflammation
Inflammation phase.. what can do done at this stage and what does it entail?
Gentle movement, help with pain and encourage/improve circulation.
Entails- vascular permability, formation of exudate -> chemical infiltration process= phagocytosis
Proliferation stage.. what can be done at this stage and what does it entail?
Continue movement and exercise, movement slow and gentle and controlled, allow collgen to be laid down along lines of stress. Too mich stress/stretch and collagen will break/fail = more inflammation.
Entails- exudate reolves, fibroblasts lay down immature collagen (Type 3) in a haphazard manner.
Remodeling.. what exercise can be done at this stage and what does it entail?
Apply stretch and stress the soft tissues, prevent haphazard scars as they prevent movement, weight bearing exercises to increae tensile strength.
Entails- Type 3 collagen changes to Type 1 -greater tensile strength.
Contraindication of general exercise?
Cogninitive impairment Acute inflammation (1st 24 hrs). Fractures (before consolidation) Severe infection/ fever Pulmonary embolism
Indication for exercise
- Muscle weakness/rebuild strength
