ESMOE - Pre-Anaesthetic Checklist Flashcards
1
Q
Main general checks:
A
- STARTING UP
- WALL
- MACHINE
- ANTI HYPOXIC DEVICE
- O2 FAIL DEVICE
- VAPORISERS
- CO2 ABSORBER
- CIRCUIT
- VENTILATOR
- AUXILLARY EQUIPMENT
2
Q
- STARTING UP
A
- Check last & next service date
- Turn on machine and monitors:
NIBP, SpO2, ECG - Calibrate O2 analyser:
FiO2 0.21 in room air, FiO2 0.90 with O2 flush - Capnograph should pick up expired breath
3
Q
- WALL
A
- Check pipelines – colours/shapes, tug test
4
Q
- MACHINE
A
- O2 cylinder should be > 5000 kPa
- Pipeline pressures should be 400-500 kPa
- The rotameters should open easily +
bobbins shouldn’t stick - When FGF is occluded, the bobbins should
bounce. (Repeat with each vaporiser open)
5
Q
- ANTI HYPOXIC DEVICE
A
- Prevents patient getting hypoxic gas mix
- The O2 bobbin should rise - when N2O is
opened - The N2O bobbin should fall - when O2 is
closed
6
Q
- O2 FAIL DEVICE
A
- Disconnect wall O2
- The O2 fail alarm should sound
- Open the O2 cylinder
- Alarm should go off
- O2 bobbin should rise
- Reattach O2 pipeline and repeat tug test
- Close O2 cylinder
7
Q
- VAPORISERS
A
- Ensure O rings (on the back bar) intact
- Ensure fitted securely and locked
- Vaporiser should be at least half full,
with the correct agent - The dials must open easily
- ONLY ONE dial should be able to open at a time
8
Q
- CO2 ABSORBER
A
- Must be securely in place
2. There should be <50% colour change - otherwise indicates absorber saturation
9
Q
- CIRCUIT
A
- No holes or cracks or water
- Switch to BAG (or manual) mode
- Check APL valve:
- 1 Close valve completely
- 2 Press O2 flush up to 40cm H2O
- 3 Compress reservoir bag
- 4 APL valve should open at 65cm H2O
- Attach test lung and ‘bag’:
- 1 Check that the tubing has no obstruction
- 2 Valves are working
10
Q
- VENTILATOR
A
- Switch onto VENTILATOR mode
- FGF should be at minimum
- Set ventilator and watch that TV
and RR that you have set is
delivered. - Bellows should rise to the top of the casing
(if not, increase FGF till bellows reach the top: this FGF is the quantity of the leak in the machine). - Acceptable leak is <200ml
- Commonest sites of leak: circuit, CO2 absorber, vaporisers and bellows
11
Q
- AUXILLARY EQUIPMENT
A
- M: Masks, Magill’s forceps
- A: Airways, Assistant
- L: LMA, Laryngoscope blades and handle
- E: ETT, Emergency drugs
- S: Stylet Strapping
- S: Suction: Should reach -30 to -50 cm H2O
- S: Scavenging
- AmbuBag: valve working, bag and tubing attached
- Defibrillator checked and available
