1. Audible stridor
2. Morbid obesity - BMI > 40; neck circumference > 50 cm
3. Limited space in mouth:
4. 1 MP score 3 or 4
5. 2 Inter-incisor distance < 3 finger-breaths
6. 3 Loose teeth
7. 4 Overbite/ prominent incisors
8. 5 Large tongue
9. 6 Excessive soft tissue - airway masses, large breasts
10. Limited joint mobility:
11. 1 Unable to protrude mandible past maxillary teeth
12. 2 TMD < 6 cm
13. 3 Unable to touch chin to chest
14. 4 Unable to hyper-extend neck (chin above occiput)
