What is ergot derivatives?
Also called ergot alkaloids. Are group of compounds originally isolated from ergot fungus, claviceps purpurea.
Out of the many ergot derivatives, just two are used as oxytocics.
State the oxytocic ergot derivatives
Ergometrine
Methylergometrine[ methergin]
State the mode of action of ergot derivatives
Acts on the directly on myometrium and excite uterine contractions
Types/preparations of ergot derivatives
- Ergometrine
0.25mg and 0.5mg ampoules - Methergin
0.2mg and 0.5-1 mg tablet
State the indications for ergot derivatives
- Therapeutic
- Prophylactic
Therapeutically ergot derivatives is used to
- Stop the atonic uterine bleeding
- Following delivery, abortion
- Expulsion of hydatidiform mole
Prophylactically ergot derivatives is used in
- Excessive haemorrhage following delivery
- Active management 3rd stage of labor
Contraindications of ergot derivatives
- Suspected plural pregnancy
- Organic cardiac disease
- Severe eclampsia and pre eclampsia
- Heart disease and HTN( hypertension)
State the side effects of ergot derivatives
- Nausea and vomiting
- Rise in blood pressure
- Stroke
- Gangrene of toe [rare]
