Anatomy?

Epistaxis = profuse - both nostrils + bleeding site cannot be identified on speculum examination - Dx? What to do?

Admit:

  • kids < ?yr #pathology
  • ?
  • comorbidities = ?
  1. ?
  • -Leaning forward = decreases BF through nasopharynx
  • minimize blood swallowing @pharynx.
  1. After 10-15mins:
    - Bleed STILL continue = ?
    - Bleed stop = ? - Avoid what? Apply what?
  2. PCR? ENT reg do what?

Posterior Bleed - ADMIT

Admit:

  • kids < 2yr #pathology
  • bleeding dx/haem-malig
  • comorbidities = HTN/CAD
  1. Hippocratic Method
    -Lean forward
    -Open-Mouth-Breathing*
    -Pinch the cartilaginous (soft) part of the nose
    #10-15mins
  • -Leaning forward = decreases BF through nasopharynx
  • minimize blood swallowing @pharynx.
2. After 10-15mins:
Bleed STILL continue = A+E
Bleed stop = Self-care: 
-Avoid Picking/Valsalva/Hot-drinks
-Naseptin / Mupirocin @peanut/soya allergy
    • Packing for <2d @cautery fail OR can’t see bleed-point #RapidRhino
    • Cautery @bleed-point seen
    • Rigid nasoendoscopy +/- Floseal
    • Reg = sphenopalatine artery ligation
