Endo - Diagnostics Flashcards
what are the general rules regarding referred pain (3)
radiates to the ipsilateral side (same)
anterior teeth rarely have referred pain to other teeth
posterior teeth refer pain to the opposite arch and periauricaular area - esp mandibular molars
what types of fibres cause referred pain?
C fibres
how do you construct a diagnosis?
Pulp diagnosis
Apical diagnosis
Describe all the relevant info from crown to apex.
what is the pulpal diagnosis for short, sharp discomfort when a stimulus is applied but only lasts a few seconds and has no significant changes on a radiograph at periapical regions?
reversible pulpits
what is the PULPAL diagnosis for sharp pain when hot stimuli is applied that lasts for >30 seconds and can also have spontaneou, referred or postural pain?
Pain not alleviated by over the counter medication
Recession also seen on a radiograph.
Symptomatic Irreversible Pulpitis
what are causes of Symptomatic Irreversible Pulpitis? (3)
deep caries
large restorations
pulpal exposure after fracture/trauma
what PULPAL diagnosis would be asymptomatic and unresponsive to sensibility testing ?
necrotic pulp
how do we manage reversible pulpits?
will resolve when the cause is eliminated i.e. caries
how do we manage irreversible pulpits?
Vital inflamed pulp cannot heal = root canal treatment required or extraction.
how do we manage pulp necrosis?
Death of the dental pulp = root canal treatment or extraction required.
what is the apical diagnosis for a tooth which has a Painful response to biting, percussion and palpitation.
possible radiolucency on radiograph.
Symptomatic Apical Periodontitis:
what is the APICAL diagnosis for a tooth which is non-vital and has signs of radiolucency/osseous destruction on a radiograph and a possible sinus tract?
Chronic Apical Abscesses - if no acute symptoms
what is the APICAL diagnosis for a tooth with spontaneous pain, Extreme tenderness, Pus formation, Swelling of associated tissues paired with systemic symptoms? (fever, malaise)
Acute Apical Abscess
what is the danger associated with Acute Apical Abscesses?
swelling of the eye and the airways
what is the apical diagnosis for a tooth which has PDL destruction/changes or radiolucency and no clinical symptoms?
asymptomatic apical periodontitis
