A

you cannot prescribe a cd 2/3 except phenobarbital for eplipsy
can only have cd 4- for a max of 5 days

    -DETAILS OF practioner
    Name, quantity and, except where it is apparent from the name, the pharmaceutical form and strength of the POM supplied
    •The name and address of the practitioner requesting the emergency supply
    •The name and address of the patient
