minor typographical error or spelling mistake
what happens when collecting S2 and S3 drugs?
must show photo ID- patient or on behalf
if healthcare prof- ask for name and address of patient and ID

chronological
how is the CD register divided?
there must be sections for each class and different parts for different strengths and forms
how long must the CD register be kept for?
kept for 2 years from the last date of entry
what form is it a legal requirement to fill when filling the CD register?

where is the home office requisition form sent to at the end of the month?

when can you supply a schedule 2/3 drug without a prescription?
in an emergency, and when it has written within 24 hours
how do you destroy out of date CDs?- managment
must be kept in seperate part of CD cabinet until destroyed
must be witnessed by an authorised witness
they will record the destroyed quantities in the CD register

how are CDs destroyed?
can be put into pharmaceutical waste once denatured
can be done with CD denaturing kit
why is it important to denature a CD
not readily retrievable without the use of specialist equipment
how to denature a solid CD
crush tablets before adding it to a denaturing kit
can add water
if not kit can place in warm soapy water- ensure drug is dissolved or dispersed
what if a CD destroying kit is not available for a liquid?
add it to an appropriate product (cat litter) and add to pharmaceutical waste bin
how are CDs parental destroyed?
opened as must as possible
amoless should be disposed of and the sharps put in the sharps bin but labeled - mixed pharmaceutical wastes and sharps- for incinuration
how are Fentanyl / Buprenorphine patches destroyed?
Just remove backing on the patch and fold the patch over on itself (sticky side inwards) and place in the CD denaturing kit
how are Aerosol formulations destroyed?
expelled into water
the resulting solution can then be disposed of in accordance with the guidance on destruction of liquid formulations
