- DR ABCDE
-haemodynamically stable
-NBM AND oxygen
Treatment of variceal haemorrhage
Prevention of haemorrhage ?
IV terlipressin + antibiotics
Terlipressin ( ADH analogue ) that causes splanchnic vasoconstriction
- Broad spec antibiotics
Prevention :
1) Propranolol
2) variceal band ligation (VBL)
Management of status epilipticus
1) IV Lorazapam ( or rectal diazepam )

3) IV PHENYTOIN
Narrow complex tachy management?
if they have asthma ?
1- vagal manoeuvres
2- IV 6MG adeNOsine
3- if asthmatic IV VERAPAMIL
Treatment of bradycardia ?
Atropine 500micrograms
VF ( chaotic ECG) / PULSLESS VT ( broad complex tachy )
- Defibrillation
- CPR
- shock x3
- Amiodarone 300mg IV + adrenaline 1:10,000
- Amiodarone is a one off dose but adrenaline can be repeated every OTHER cycle )
Treatment of acute angle closure glaucoma ?
- features
- treatment
red painful eye , dilated pupils , haloes and visual impairment
- raised IOP
- Corneal oedema
Treatment :
1) Beta blockers + pilocarpine or IV acetazolamide ( to lower IOP)
2) Surgical : Peripheral Iridotomy , surgical iridectomy
Extradural haemorrhage (Epidural haemorrhage)
signs and treatment
- Epidural = damage to the middle meningeal veins , pie on CT
- lucid intervals
CT SCAN shows the pie / mid line shift
Subdural haemorrhage signs
banana on CT
- consciousness varies
- memory impairment
- older people with minor head injury
Necrotising fascitis features
pain out of proportion to what is observed + systemic illness ( difficult to diff between cellulitis )
++ T2DM is a risk factor
Treatment of encephalitis
IV Ceftraixone + IV Acyclovir
treatment for pulmonary oedema
sit patient up
IV furosemide
