Inflammation d/t fissures in articular cartilage and bone, fragmentation

A
UNCOMMON
Adolescent athletes!
M>F (10:1); javelin/pitchers; female gymasts
MC site of OCD in UE

How does it happen, yo?
A
Repetitive valgus stress pressing against capitellum
Medial epicondylosis
Instability

What’s it look like?
A
Gradual onset! Lateral elbow pain with or without creptitis Decreased AROM ESPECIALLY IN EXT Tender, large radial head Lateral elbow pain with valgus

What tests should i do?
A
Xray, depending on severity, mgt and prognosis

Early treatment
A
POLICE: rest (months), sling
AVOID MANIP
STM, EMS, hand exercises if post surg or w/o fragmentation

Late tx?
A
Adjust through pain, use caution with da young boys
