Morphology
RAA=tall, peaked P waves in II
LAA = notched P waves in II, wide and deep terminal forces in V1
Vector(varies)
Wider than 1 little square or deeper than 1 little square
What are the causes of Ventricular enlargement?
Pressure overload or volume overload
With Atrial enlargement
Either or both ventricles
called “Ventricular hypertrophy” even though the wall might not be thickened
(Wall will thicken until it can’t anymore then it dilates)
What are the Features of RVH?
QRS normal
Late intrinsicoid deflection in V1-2
Right axid deviation
ST depression with upward convexity and inverted T waves in right precordial leads
What does tall, peaked P wave tell you?
Atrial conduction abnormalities
What tells you there is a 1st degree AV block?
PR longer than it should be
What is the difference between LV Dilation vs Hypertrophy?
Dilation: volume overload compensation or failed hypertrophy
Hypertrophy: from pressure overload (aortic stenosis, or systemic HTN)
What is the approach to analysis of LV enlargement.
Duration: QRS can be prolonged
Amplitude: numerous criteria
Morphology: Eval QRS for late intrinsicoid deflection and ST seg for depression
Vector: QRS axis in 2 planes
What does inverted T waves tell you?
ventricular repolarization abnormality (LVH)
