Define Refeeding syndrome

A

any route after a prolonged period of starvation.

What is the pathophysiology of Refeeding syndrome?

A

Pt has ↓food which causes ↓insulin. Fat/Protein catabolism occurs and intracellular PO4↓
When Pt is refed ↑insulin due to ↑carbs so PO4 uptake into cells ↑ → hypophosphataemia

A
Rhabdomyolysis
Respiratory insufficiency
Arrhythmias
Shock
Seizures 
Due to ↓K, ↓Mg, ↓PO4

How do you manage Refeeding syndrome?

A

Identify at risk patients in advance - liase with dietician
Parenteral and oral PO4 supplementation
Manage any complications
