A

Using tunning fork to evaluate sound waves through bone

Placed on the patient’s forehead and ask the patient which side they hear the most

Rhine Test

A

Compares air conduction to bone conduction

Placed on mastoid bone and the patient will hear the tunning sound. After it fades, place the tunning fork by client’s ear.

Inspection of nose

A

Nose is aligned, central and symmetrical with two nares. Cilia present.

Skin consistent with the rest of the body

No lesions, lacerations or lesions noted

Pink mucous membranes

No drainage/discharge

A

Mouth is central and pink in color. No ulcers, scaliness, cracking or ulcers noted. Mucous membranes of are pink, healthy and moist. Uvula is midline in the back of the mouth.

Client has all 32 teeth without cavities.

Palpation of mouth

A

With gloves, tongue is moist and warm with papillae.

Inspection of Neck

A

Neck is central and normal complexion in comparison to the rest of the body. No lesions, masses laceration or lumps noted. Trachea appears midline.

Palpation of neck

A

Thyroid felt to be midline. Lymph nodes unpalpable without any nodules. Carotid artery 3+ with regular rate and rhythm.

A

The sound of blood flowing through a narrowed portion of an artery
