difficulty producing sound with change in voice pitch or quality
when should you investigate
if lasting >3 weeks as chief presentation. suspect laryngeal carcinoma
tests
laryngoscopy
specific causes
reflux laryngitis, laryngitis, reinkes oedema
what is reflux laryngitis assoc with
GORD- give PPI
what is laryngitis
usually viral and self limiting. can be secondary to GORD or autoimmune disease.
symptoms laryngitis
pain, hoarse, fever. give penicillin if necessary
what is reinkes oedema
chronic cord irritation from smoking +- chronic voice abuse,
less common causes dysphonia
GBS, goitre, carotid body tumour, neoplasm, toxic, epiglottitis, abscess, decr lubrication, layngoceles
what is laryngeal nerve palsy
vocal cord paralysis symptoms- hoarse with breathy voice, and weak cough; repeated coughing/aspiration; exertional dyspnoea.
causes laryngeal nerve palsy
cancers, iatrogenic, CNS disease, TB, AAA
