what is the term for painful swallowing
odynophagia
what is globus pharyngeus
sensation of a lump in the throat when not swallowing food with no primary swallowing difficulty
fluid can be drunk as fast as usual except if food is stuck
yes- stricture. no- motility disorder
difficult to make the swallowing movement
yes- bulbar palsy- suspet especially if coughs on swallowing
is dysphagia constant and painful
yes- suspect malignant stricture
does the neck bulge or gurgle on drinking
yes-pharyngeal pouch
malignant causes dysphagia
oesophageal cancer, pharyngeal, gastric, extrinsic pressure eg lung cancer
neuro causes dysphagia
bulbar palsy, lateral medullary syndrome, MG, syringomelia
other causes dysphagia
benign strictures, pharyngeal pouch, achalasia, systemic sclerosis, oesophagitis, iron def anaemia
tests
FBC,ESR, barium swallow, endoscopy with biopsy, oesophageal motility studies, CXR
causes benign oes stricture
GORD, swallowing corrosives, FB, trauma
pharyngeal pouch symptoms
dysphagia, choking, chronic cough, aspiration, regurgitation, halitosis. gurgles
treatment pharyngeal pouch
endoscopic stapling
