Dyshemoglobinemia Flashcards
Why do we use hyperbaric oxygen?
Prevents lipid peroxidation and delayed neurologic sequelae in CO poisoning
Why are nitrite dangerous?
Turns RBC hemoglobin to methemoglobin
CN moves from Fe3+ on cytochromes to those in RBCs creating CYANOMETHEMOGLOBIN
Nitrite poisoning is particularly dangerous if there is a concurrent?
CO poisioning
How does sodium thiosulfate work?
Enhances normal metabolism of cyanide via rhodanase
What is hydroxycobalamin used to treat?
Binds with cyanide –> cyanocobalamin(B12)
When should hydroxycobalamin be used?
Smoke inhalation victims not improving with supportive care
Cyanide exposure
Nitroprusside at risk patients
What is the mechanism for methylene blue?
Cofactor of NADPH reductase
Gains electron, donates directly to reduce methemoglobin
When is methylene blue useful?
In methemoglobinemia >20-30% or with symptoms
What are side effects of methylene blue?
Hemolyctic anemia(weak oxidizing capability)
