Acronym for DVT?
THRAMBOSIS (Increased risk) T - Trauma, travel H - Hypercoagulable, Hormones R - Recreational IV drugs A - Age > 60 M - Malignancy B - Birth control O - obese, obstetrics S - surgery, smoking I - immobilization S - Sickness
Virchow’s triad?
(a) Venous stasis
(b) Hypercoagulable state
(c) Injury to vessel wall
Symptoms of DVT?
(1) Pain, redness, swelling, warmth, and tenderness
(2) Measure leg circumference and compare to unaffected leg
(3) If unilateral leg swelling then you MUST rule out DVT, or transfer to somewhere that can
(4) Homan’s sign (pain with dorsiflexion) is unreliable sign for DVT
Labs/Imaging for DVT?
D-dimer
Ultrasound (ALL PATIENTS)
Tx for DVT?
- Aggressive anticoagulation with Lovenox 1mg/kg SC q 12 hours
Initial care for DVT?
ABCs, IV, O2
Anticoagulant
MEDEVAC
Where does the embolism form in PE?
Pulmonary artery - this puts stress on right ventricle
Signs of PE?
Hypoxic
SOB
Chest pain, tachypnea, tachycardia
Signs of DVT
Imaging study of choice for PE?
Helical CT pulmonary
US for lower extremities
Tx for PE?
(1) ABCs, IV, O2
(2) Anticoagulate
(3) MEDEVAC
