What is a critical defect?
One which can adversely impact the safe operation of the train and/or train performance.
When should you report a critical train defect and to whom?
- immediately to the signaller
- within 2 minutes to DFM
What are non critical train defects?
Defects which will not impact the safe operation of the train and/or train performance.
How would you report a non-critical defect?
- report to DFM at first convenient opportunity
- enter defect into repair book at end of journey
What is the DOTE plan?
A documented process to manage train defects on the job (before, commencing, during) to ensure the safe operation of a train
Speed restrictions for defective AWS/TPWS
- w/ competent person who has driver route knowledge: line speed
- w/o competent person: 60
- poor visibility: 40
Speed restrictions for defective vigilance
- w/ competent person: line speed
- w/o competent person: 60
- competent person must know how to stop train in emergency
Speed restriction for complete headlight failure
- 20
- 75 with x3
- competent person not required
Speed restriction for obscured/broken windscreen
- w/ competent person with drivers route knowledge: line speed if safe to do so
- w/o competent person: safe speed, sounding horn more frequently
Speed restriction for complete horn failure
- 20 (line speed for partial failure)
- competent person not required
Speed restriction for confirmed hot axle box
- 10 / 5 over points
- competent person not required
Speed restriction for GSM-R failure
- 100 for on board failure
- 60 for network failure
Who can be a competent person with drivers route knowledge?
- qualified driver
- DI or SDI
- DM
- Driver Safety Assurance Manager
- Operations Standards Manager
Must have route knowledge!
What must you do as a competent person for isolated AWS on approach to signals
- reach a clear understanding on how to communicate signal aspects, verbalise cautionary or danger aspects
- observe and acknowledge the drivers reaction to cautionary and danger aspects
- if necessary, remind the driver of cautionary and danger aspects
- take appropriate action if the driver doesn’t react to cautionary aspects (emergency brake)
As a competent person for isolated AWS, what must you do on approach to speed changes
- reach a clear understanding on how to communicate the changes
- acknowledge the drivers reaction
- if necessary, remind the driver
- take appropriate action if the driver doesn’t respond
