childhood onset fluency disorder (stuttering)

A

Disturbances in the normal fluency and time patterning of speech that are inappropriate for age and language skills; persistent over time and are characterized by one or more of the following:
Sound and syllable repetitions
Sound prolongations of consonants as well as vowels.
Broken words
Audible or silent blocked (filled/unfilled pauses)
Circumlocutions (word substitutions)_
Words produced with an excess of of physical tension
Monosyllabic whole word repetitions
Disturbance causes anxiety about speaking or limits effective communication
Onset of symptoms is in early development
Not attributed to speech/motor or sensory deficit.
Onset occurs from age 2-7

Autism Spectrum Disorder

A

Persistent impairment in reciprocal social communication and social interaction, restricted, repetitive patterns of behavior, interests or activities. Symptoms are present from early childhood and limit or impair everyday function.

ADHD (Inattentive)

A

A persistent pattern of inattention and/or hyperactivity-impulsivity that interferes with functioning or development, as characterized by:
Six or more symptoms that have persisted for at least six months to a degree that is inconsistent with developmental levels and negatively impacts activities. For older adults (17+) only 5 symptoms are required.
IE fails to give close attention, difficulty sustaining tasks, does not seem to listen or follow through on instruction

ADHD (Hyperactive/impulsive)

A

Six or more symptoms that have persisted for at least six months to a degree that is inconsistent with developmental levels and negatively impacts activities.
IE “on the go”, fidgets, leaves seat when it is inappropriate, runs or climbs, talks excessively
3) Several symptoms were present prior to age 12
4) Several symptoms present in 2 or more settings

A

Presence of two or more for a significant portion during a 1 month period.
Hallucinations
Delusions
Disorganized Speech
Grossly disorganized or catatonic behavior

*Brief psychotic disorders lasts more than 1 day and remits by month.

Manic Episode

A

A distinct period of abnormally and persistently elevated, expansive or irritable mood and abnormally and persistently increased activity. Lasts at least 1 week and present most of the day, nearly every day.
**Three or more of the following:
Inflated self esteem or grandiosity
Decreased need for sleep
More talkative then normal
Flight of ideas/racing thoughts
Distractibility
increased in goal directed activity

	****Causes marked impairment, not attributable to substances or another 				medical condition, or psychotic features are present. 
	**Predominate mood is irritable rather than euphoric.

Hypomanic Episode

A

All criteria for manic episode, then:
**associated with an unequivocal change in functioning that is uncharacteristic of the individual.
**Disturbance in mood/change in function are observable by others
**IS NOT severed enough to cause naked impairment in social/ occupational functioning
**WITHOUT psychotic features.
**Common in bipolar one dx, but not required.

Bipolar Type 1

A

**Must meet criteria for a manic episode. May have been preceded by and may be followed by a hypomanic/major depressive disorder

Major Depressive Disorder

A

**Five or more of the following symptoms during the same 2 week period and represent a change from previous functioning.
**
At least one symptom MUST be depressed more or loss of interest/ pleasure
1) Depressed mood, most of the day, nearly every day.
2) Marked diminished interest or pleasure in (almost) all activities most of the day, nearly everyday.
3) Decreased appetite/significant weight loss (more than 5%)
4) insomnia/hypersomnia
5) psychomotor agitation
6) fatigue/loss of energy
7) Feelings of worthlessness
8) Diminished ability to think/concentrate
9) Recurrent thoughts of death.
Common in Bipolar 1 but not required

Bipolar 2

A

Presence of hypomanic episode AND presence of current or past major depressive episode
*Has never been a manic episode
*common feature is impulsivity, which can increase suicide attempts.
*prevelance: 0.8%
*5-15% will develop a full manic episode, and therefore bipolar 1

Cyclothymic Disorder

A

*Numerous periods with hypomanic symptoms (do not meet criteria for hypomanic episode) for at least 2 years (1 in children)
*Numerous periods with depressive episode symptoms that do not meet full criteria for at least 2 (1) years.
*Symptoms present for at least half the time, and has not been without symptoms for more than 2 months at a time.

Disruptive Mood Dysregulation Disorder

A

Chronic, severe persistent irritability.
two prominent clinical manifestations:
1) frequent temper outbursts - typically occur in response to frustrations (verbal or behavioral - aggression against property. Self or others)
2) occur three or more times a week over at least one year in at least two settings.
Severe recurrent temper outbursts (verbal or behavioral) that are grossly out of proportion.
Outbursts are in consistent with developmental level
occur on average, three or more days a week.
Mood between outbursts is persistently irritable or angry, most of the day, nearly everyday
*Criteria A-D have been present for 12 or more months, without a period of 3 or more months with no symptoms
*Criteria A-D are present in at least two of three settings, and severe in at least one of them
*diagnsoed between ages 6-18
*onset is before 10 years old

Persistent Depressive Disorder

A

consolidates major depressive disorder and dysthymic disorder from DSM 4
1)Depressed mode for most of the day, for more days than not for at least 2 years.
2) Presence of two or more:
A)poor appetite/overeating
B)insomnia/hypersomnia
C)low energy or fatigue
D)low self esteem
E)poor concentration or difficulty making decisions
F)feelings of hopelessness

Individual has never been without symptoms for more than two months at a 			time. 
Has never had manic episode
**Can be diagnosed at the same time as major depressive disorder - would be separate diagnosis.
