• loading dose: 10,000-15,000 units
• maintenance dose via IV drip: start with 1,000 u/hr
• daily monitoring of PTT required, titrate heparin to maintain 1.5 x control

5 mg PO QD (can do 2.5-15 mg/day) – titrate to INR 2-3

*note peak effect in 3-5 days (so want to start heparin initially at same time and then stop once reach INR anticoag dose)

Drugs for P. Neuropathy

1. Neurontin (Gabapentin)
   - 300 mg at least TID (min dose = 900 mg/day; max dose = 3600 mg/day)
2. Lyrica (Pregabalin)
   - 50 mg 1 PO TID
3. Cymbalta (Duloxetine)
   - 60 mg I PO QD
   - don’t give to pts with glaucoma or possible psych issue (depression)… which is many of our dm pts
