What is an allergic reaction?
Independent of dose and may persist long after drug has been stopped - more severe if drug given again
What is a toxic reaction?
Depends on dose - reactions subside when drug is stopped
What causes drug induced skin reactions?
- Genetic factors
- Patients with hepatic, renal, SLE and AIDs - increased risk
- Route of administration
How would you diagnose drug induced skin reactions?
Timing of skin reactions - more than one drug makes it difficult
What is an erythematous reaction?
- Erythema - maculopapapular (rash with both raised and flat parts) or mobiliform (measle like rash)
- Affects whole skin except face usually
- Cause skin to become red, hot, burny and itchy
- Most common
What drugs cause erythematous skin reaction?
- Penicillins
- Cephalosporins
- Sulphonamides
- Thiazides
- Gold
- Furosemide
- Anti TB drugs
- Phenothiazines
What is a fixed drug eruption?
- An erythematous, dusky purple round or oval lesion with blisters or vesicles
- Initially one lesion - then others
- Appears within 24 hours of ingestion
- Commonly affects hands , feet, penis, perinatal area but can be anywhere
- Site is fixed
- Drugs are the sole cause
What drugs caused a fixed drug eruption?
Sulphonamides and barbiturates
What is acute uticaria and angiodema?
Nettle rash - 36 hours after ingestion
Angiodema - vascular reaction causing increased permeability and fluid leakage
Stop drug and give antihistamine
Report to MHRA
Where does uticaria and angiodema affect?
Tongue, lips, eyelids and genitalia
URT - can cause death
Unilateral or symmetrical
What drugs cause uticaria/angiodema?
- Ace inhibitors
- Anaesthetics
- Antibiotics
- Dextrans
- Enzymes
- NSAID’s and opioids
- Hydralazine
- Radio-contrast media
What is Steven Johnson syndrome?
Extensive erythema multiforme of face and trunk causing fever, malaise, myalgia and arthralgia
Stop - give fluids and systemic steroids
What drugs cause Steven Johnson syndrome?
- Allopurinol
- Carbamazepine
- Lamotrigine
- Penicillins
- Tetracyclines
- Sulphonamides
- NSAID’s
What is a photosensitivity reaction?
Phototoxic - exaggerated sunburn, weeping and desquamation
Rash confined to area exposed to light
What causes photosensitivity?
High doses of drug and sufficient light
Amiodarone
