GCS
Glasgow Coma Scale.
Objectively describes extend of impaired consciousness.
Eye-opening, motor and verbal responses.
Not as complete as Four but more practical.
FOUR
Full outline of unresponsiveness rating scale.
Developed because verbal response cannot be assessed in intubated patients. Makes a difference between UCS and MCS, through visual pursuit.
Eye response, motor response, brainstem reflexes, respiration.
CRS-R
Coma Recovery scale.
Assesses auditory, visual, verbal and motor responses, brainstem reflexes, arousal and communication.
CN1 test (olfactory)
Inoversity of pennsylvania smell identification test (or observation)
CN2
Pupil size, shape and symmetry.
Swinging light test.
CN3,4,6
Swinging light test.
Accommodation reflex.
Ptosis.
Eye pursuit.
Cover test.
CN5
LTT and PPT on forehead (ophthalmic branch), cheek (maxillary branch) and jaw (mandibular branch).
Jaw jerk refelx.
Corneal reflex.

- Observation of forehead wrinkles, nasolabial folds and mouth symmetry. Movements: raised CN7 eyebrows, closed eyes, blown out cheeks, smiling, pursed lips, closed lips, angry face.

Gross hearing test

Gag reflex. observe uvula symmetry (deviation to one side, deficit of opposite cranial nerve), cough and swallow.
CN11
Resisted shoulder elevation.
CN12
Observation protrude tongue (symmetry)
Visual Field Tests
CN2,3,4,6.
Confrontational visual test: assesses differences in visual field (sensory and/or perceptive) on the right and left side.
Automated perimetry test.
