Why are glucocorticoids used as DMARD?
Induce remission while waiting for slower-acting DMARD to work. Short courses during flare-ups
DMARD of choice in rheumatoid arthritis
Methotrexate
Methotrexate - MOA
Inhibition of human folate reductase.
Inhibition of lymphocyte proliferation & production of cytokines & rheumatoid factor.
Interferes with polymorphonuclear leukocyte chemotaxis, which reduces cytotoxins & free radicals
Methotrexate - contraindications
Pregnancy
Methotrexate - adverse effects
Hepatic & pulmonary reactions
Immunosuppressive drug used as DMARD
Leflunomide
Leflunomide - MOA
Inhibits leukocyte & T-cell proliferation by inhibiting pyrimidine synthesis in immune cells
Leflunomide - Contraindications
Pregnancy
Leflunomide - adverse effects
Reversible alopecia
Methotrexate cannot be combined with
Leflunomide
Antimalarial drug used as DMARD
Hydroxychloroquine
Hydroxychloroquine - adverse effects
Blurred vision
Scotomas
Night blindness
Hydroxychloroquine - MOA as DMARD
Reduces chemotaxis and phagocytosis of PMNs, reduced superoxides from these cells
