Cardiac Glycoside, inotropic agent
Indications for Digoxin
Congestive heart failure, re-entry SVT, ventricular rate control in atrial flutter and atrial fibrillation
Mechanism of action for digoxin
Rapid-acting cardiac glycoside with direct and indirect effects: increases force of myocardial contraction, increases refractory period of AV node, and increases total peripheral resistance
Contraindications for Digoxin
Ventricular fibrillation, ventricular tachycardia, digitalis toxicity, hypersensitivity to digoxin
Adverse effects of Digoxin
Fatigue, headache, blurred yellow or green vision, seizures, confusion, bradycardia, dysrhythmia, nausea, vomiting, anorexia, skin rash
Precautions for Digoxin
Has a narrow therapeutic range. Amiodarone, Quinidine and Verapimil will increase serum Dig levels up to 70%, use with diuretics may Potentiate cardiac toxicity
Adult Dose of Digoxin
loading dose 4 to 6 mcg/kg over 5 minutes. 2nd and 3rd bolus of 2 to 3 mcg/kg to follow at 4 to 8 hour intervals
