sulfonylureas
elderly, decreased renal fx for those with active metabolites
Metformin
Renal insufficiency, (GFR 1.4 for women or 1.5 for men), Age > 80, CHF, COPD, w/i 48 hours of radiocontrast die
TZDs
liver disease or ALT > 2.5 ULN, NYHA class 3 or 4.
Meglitinides
caution in renal and hepatic impairment
Alpha-glucosidase inhibitors
patients with short-bowel syndrome or inflammatory bowel disease, patients with Scr >2
DPP IV
Pancreatitis, DKA, HF, hepatic/ renal impairment. use w/ insulin may increase hypoglycemia
SGTs
GFR <30, ESRD, dialysis, DKA
Bile Acide Sequestrants
PMH of bowel obstruction or hypertriglyceridemia induced pancreatitis, TG> 500 mg/dl
Dopamine Agonists
Pediatrics, pregnancy, combo with other hypoglycemic agents may increase risk of hypoglycemia
