Chlorpropamide
250mg PO QD, may increase by 50-125 mg at 3-5 day intervals.
Glipizide
5mg PO QD initially. Titrate in 2.5-5 mg increments every few days. Max dose: 40 mg/day
Glyburide
2.5-5mg PO QD. increase up to 2.5mg/day at weeky intervals. Max dose: 20mg/day
Glimeperide
1-2 mg PO QD, may increase 1-2 mg q 1-2 weeks. Max: 8mg/day
Metformin
500 mg BID, increased by 500 mg weekly until glycemic goals OR 2,000 mg/day
Pioglitazone
15-30 mg QD
Rosiglitazone
2-4 mg QD
Acarbose and Miglitol (dosing is the same)
25 mg with one meal a day, increase gradually over several months to a max of 50mg TID if < 60 kg, or 100mg TID if >60 kg.
Lantus and Levemir
Starting doses:
Type 1: .2-.4 U/kg
Type 2: 1 U per 20mg/dl above 100 fasting, 2 U per 20 mg/dl above 140 fasting
Humalog, Novolog, Apidra
1 U per 12-15 grams of carbs
Pramlintide
Type 1: 15 mcg prior to meals, can be titrated up in 15 mcg increments to a max of 60 mcg
Type 2: 60 mcg prior to meals, may be titrated to a max of 120 mcg.
Exenatide
5 mcg BID, titrated to 10 mcg BID in 1 month, or as tolerability allows.
Liraglutide
.6mg daily for >1 week, then 1.2 mg daily for > 1 week. pts may be maintained on the 1.2 mg dose or increased to the max 1.8 mg daily dose after > 1 week.
Sitagliptin (Januvia)
100 mg PO QD
Saxagliptin (Onglyza)
2.5-5 mg QD
