What?
Polymyositis with additional typical cutaneous features
Typical features?
Gottron’s sign
Hellotrope rash
Shawl sign
Gottron’s sign?
Reddish/ purplish discolouration of PIP and DIP joints
Hellotrope rash?
Similar to Gottrons but around the eyes
Eye swelling
Shawl sign?
Reddish/ purplish rash on back of shoulder & front of chest
Examination?
Look at muscles -> wasting?
Skin involvement> - for dermatomyositis
Test muscle power – confrontational testing, isotonic testing
Investigations?
Raised inflammatory markers Raised serum CK levels Autoantibodies MRI EMG Muscle biopsy
Autoantibodies?
ANA, anti- Jo- 1, anti- SRP
Definitive investigation?
Muscle biopsy
Management?
Prednisolone (40mg) with immunosuppressive drug (methotrexate or azathioprine)
What is this associated with?
Malignant (must be screened for once diagnosed)
