How do you treat lice?
Mechanical removal is key. Do at least 2 permethrin treatments (itch isn’t a sign of treatment failure) if 1 doesn’t work.
Oral ivermectin is last line therapy.
What to know about corticosteroids and psoriasis?
Cornerstone of treatment particulary with limnited disease, can be combined with topical agents, UV light, and systemic agents.
Some degree of rebound when stopping.
What to know about calcipotriene and plaque psoriasis?
Used in plaque type psoriasis. Associated with burning, pruritus, edema, peeling, dryness, etc. has to be applied after UVA.
What to know about tazarotene?
Plaque type use, pregnancy category x
What to know about anthralin?
T-lymphocyte activation, apply with caution to face and intertriginous areas
Can coal tar cause odor and staining?
YES
What is the most common systemic treatment for psoriasis?
Methotrexate
What to know about methotrexate?
Folate supplementation required except on the day of methotrexate, conception should be avoided at least 3 months in men and 3 ovulatory cycles in women
What to know about cyclosporine?
Severe, recalcitrant psoriasis. Hypertension is a common adverse effect (reduce 25-50%). Monitor for nephrotoxicity.
What to know about Acitretin?
Severe plaque type psoriasis
3-6 months for maximal response
Alcohol ingestion can transform acitretin (can take 3 years to eliminate from the body)
Pregnancy category X
Can cause increase in triglycerides
What to know about etanercept, infliximab, and adalimumab?
Moderate to severe psoriasis and psoriatic arthritis. Painful injection site reactions in up to 15%, PPD required, NY 3 or 4 HF should be avoided.
How does benzoyl peroxide work?
Releases oxygen free radicals, reduces FFA levels by 50% and P.acnes by 98% after 2 weeks.
What is the preferred antibiotic(s) for acne?
Clindamycin for topical and tetracycline for systemic
What to know about oral retinoids?
After 16 weeks of therapy and see a 70% success that can last > 20 months. Can cause photosensitivity (sunscreen is recommended), Category X medication.
