Dentine Hypersensitivity Flashcards
what is the composition of dentine?
70% inorganic hydroxyapatite
20% organic - collagen
10% water
what does dentine contain?
collagen
mucopolysaccharides
process of the odontoblasts
where do odontoblasts lie?
at the periphery of the pulp, process extended to the ADJ
what to tubules allow?
allow passage of fluid, chemical agents and bacteria
how many mm2 of tubules at the ADJ? how many closer to pulp?
ADJ - 20 000m2
closer to pulp - 65 000mm2
what is secondary dentine?
dentine produced after the tooth is fully formed and slowly throughout life. Deposited on floor and root of pulp chamber
what is tertiary/reparative dentine?
when dentine produced in response to a noxious stimulus that cannot be explained as arising from any other form of dental defect/pathology
sensitive dentine has how many more times of open tubules at the surface? how much bigger are they?
8 x number of
2 x the diameter
what can hypersensitivity be caused by?
removal of enamel/dennudation of rooth surface
what can cause removal of enamel?
attrition/erosion/abrasion/caries
what can cause dennudation of the root surface?
gingival recession
chronic perio disease
perio surgery
trauma
what is pain felt on in hypersensitive patients?
hot/cold/mech/probe/removal of water/absorbant materials
is dentine uniformly sensitive?
no
where is dentine most sensitive?
at dentinoenamel junction and close to the pulp
if pulp inflamed = increased sensitivity
what is the dentine receptor mechanism?
suggests odontoblasts have sensory function. when stimulated odontoblasts communicate to pulpal nerve plexus resulting in pain
