A legal document on which a dentist reports to an insurance carrier the service provided for an enrollee. Information on a claim form includes a description of the service, procedure code, date of service and the fee. The dentist is responsible for the accuracy of all information on a claim form. Claim forms can ne submityed on paper, electromically, or via Delta Rentals website.
